Job Title: Clinical Care Manager - Peritoneal Dialysis

We are seeking a skilled and dedicated professional who can provide comprehensive patient care in an independent setting, empowering patients to manage their own health.

This role involves leading, coaching, and guiding patients, support systems, and care partners to ensure optimal treatment outcomes.

Key Responsibilities:
  • Develop and implement complete care plans for patients undergoing peritoneal dialysis
  • Educate patients on treatment options and provide ongoing support and guidance
  • Collaborate with healthcare teams to ensure seamless care coordination

Requirements:
  • Current Registered Nurse (RN) license in the state of practice
  • Minimum 12 months of RN experience
  • Associates Degree in Nursing (A.D.N) from accredited school of nursing required; Bachelor of Science in Nursing (B.S.N); three-year diploma from accredited diploma program may be substituted for nursing degree
