About the Role

We are seeking a skilled Physical Therapist to join our Home Health team at AccentCare. As a Physical Therapist, you will play a vital role in providing high-quality patient care and improving the quality of life for our patients.

Responsibilities
  • Provide physical therapy services to patients in their homes, focusing on improving mobility, strength, and function.
  • Develop and implement individualized treatment plans to meet the unique needs of each patient.
  • Collaborate with other healthcare professionals to ensure comprehensive care and optimal patient outcomes.
  • Conduct thorough assessments and evaluations to identify patient needs and goals.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
Requirements
  • Master's degree in Physical Therapy from an accredited program.
  • Current licensure to practice as a Physical Therapist in the state of operation.
  • Valid CPR certification.
  • Minimum 9 months of experience as a Physical Therapist, with preferred experience in home health or community settings.
