Job Summary:
We are seeking a highly skilled Oncology Nurse Clinician to join our team at Northwestern Memorial Healthcare. The successful candidate will be responsible for coordinating multidisciplinary care for patients with cancer, working closely with physicians, nurses, and other healthcare professionals to ensure comprehensive and effective treatment plans.
Responsibilities:
* Assess and reassess patients and families for ongoing care needs, identifying high-risk criteria and initiating referrals to appropriate resources in a timely manner
* Collect and analyze data, identifying and coordinating appropriate multidisciplinary care and appointments
* Collaborate with patients, families, physicians, and ancillary staff to coordinate new patients for multidisciplinary clinics
* Assist in creating a plan of care that indicates collaboration and communication
* Serve as a resource to non-clinical staff regarding prospective patients calling the program for information
* Perform all aspects of patient care in a manner that optimizes patient safety and reduces the likelihood of medical/healthcare errors
* Identify learning needs, abilities, preferences, and readiness; assess support systems, coping mechanisms, and barriers to learning, making referrals as appropriate
* Educate patients, families, and caregivers regarding diagnosis, process, therapies, and testing
* Assist with transitions of care and providers
* Communicate effectively and work cooperatively with other team members
* Delegate appropriately to other care providers to facilitate meeting patient needs in a timely manner
* Promote a positive work environment
* Demonstrate an interest in personal development and growth beyond routine expected educational sessions
* Embrace new employees and care team members, contributing to their growth and learning
* Attain knowledge and competency that reflects current nursing practice through participation in ongoing educational activities
* Engage in a formal process seeking feedback regarding one's own practice from peers and professional colleagues
* Exhibit creativity and flexibility through the dynamics of change
Qualifications:
* Completion of an accredited Registered Nurse program with an RN License in the state of Illinois
* Current nursing experience within the past 5 years
* BLS Certification
* Bachelor of Science in Nursing (preferred)
* Oncology Certified Nurse (preferred)
