Job Summary

We are seeking a highly motivated and compassionate Community Health Worker to join our team in Lexington, KY. As a Field-Based Health Advocate, you will play a vital role in assessing, planning, and implementing care strategies that improve the health and well-being of our members.

Key Responsibilities
  • Assess and identify the needs of our members, including social determinants of health such as transportation, housing, and food access.
  • Develop and implement individualized care plans that address the unique needs of each member.
  • Collaborate with healthcare providers, community organizations, and other stakeholders to ensure seamless care coordination.
  • Provide education and support to members on community resources and benefits.
  • Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services.
Requirements
  • High School Diploma or equivalent required; Bachelor's degree in a health-related field or social work preferred.
  • 2+ years of experience working in a community health setting, with a focus on case management and care coordination.
  • Strong communication and customer service skills, with the ability to work independently and maintain good judgment and accountability.
  • Intermediate computer proficiency, including MS Word, Excel, and Outlook, and ability to use multiple web applications.
  • Ability to travel locally approximately 50% of the time and up to 100 miles round trip with reliable transportation.
