Job Title: Occupational Therapist Assistant - Home Health

Puget Sound Home Health & Hospice is a Medicare-certified Home Health agency seeking a passionate and dedicated Certified Occupational Therapist Assistant to join our team.

Job Summary:

We are looking for a highly skilled and compassionate Certified Occupational Therapist Assistant to provide high-quality patient care and support to our patients in their homes. As a Certified Occupational Therapist Assistant, you will work closely with our registered occupational therapists to develop and implement individualized treatment plans that promote patient independence and well-being.

Responsibilities:
  • Participate in the ongoing evaluation of patient's functional status and home environment to identify potential hazards or barriers to independent living.
  • Teach new skills or retrain patients in daily activities that have been lost due to illness or injury.
  • Maintain accurate and detailed clinical records, including clinical notes and reports to the registered occupational therapist.
  • Submit clinical documentation in accordance with organization policy.
  • Follow treatment programs and goals for improved patient function as established by the registered occupational therapist.
  • Maintain contact and communication with other personnel involved in patient care to promote coordinated and efficient care.
  • Attend and contribute to in-services, case conferences, and other meetings as required by organization policy.
  • Identify patient and family/caregiver needs for other home care services and consult with the supervising registered occupational therapist as necessary.
Requirements:
  • Graduate of an occupational therapy assistant curriculum accredited by the American Occupational Therapy Association.
  • Certified with the National Certification Examination of the American Occupational Therapy Association.
  • Licensed by the State Board of Healing Arts in accordance with State licensure laws.
  • A minimum of one year of clinical experience as a certified occupational therapy assistant.
  • Demonstrates good verbal and written communication, and organization skills.
  • Possesses and maintains current CPR certification.
