We are seeking a highly skilled Certified Occupational Therapist Assistant to join our team at Reliant - Avalon Health Care - Tacoma. As a Certified Occupational Therapist Assistant, you will play a vital role in providing exceptional care to our patients in a Long Term Care and Skilled Nursing Facility setting.
Job Summary:The ideal candidate will possess a current Washington license/certification and at least two years of recent experience as a Certified Occupational Therapist Assistant in LTC and SNF settings. You will be responsible for delivering high-quality care to our patients, working collaboratively with our interdisciplinary team to achieve optimal outcomes.
Responsibilities:- Provide direct patient care services, including assessments, interventions, and evaluations.
- Develop and implement individualized treatment plans to meet the unique needs of each patient.
- Collaborate with the healthcare team to ensure seamless care transitions and optimal patient outcomes.
- Conduct thorough assessments and evaluations to identify patient needs and develop effective treatment plans.
- Communicate effectively with patients, families, and healthcare team members to ensure comprehensive care.
- Current Washington license/certification as a Certified Occupational Therapist Assistant.
- At least two years of recent experience as a Certified Occupational Therapist Assistant in LTC and SNF settings.
- Current BLS and/or ACLS and/or Specialty Certifications.
- 2 current Clinical References.
- Physical and TB Records.
- Tdap/Flu vaccinations and Titers.
- MMR/Hep B/Varicella or vaccination records.
- Must be able to pass background/urine drug screening upon accepting an offer.
