Job Title: Community Health Worker

At Jefferson Health, we are seeking a highly motivated and compassionate Community Health Worker to join our team. As a Community Health Worker, you will play a vital role in helping individuals and communities adopt healthy behaviors and navigate the healthcare system.

Key Responsibilities:
  • Effectively communicate and navigate systems to ensure patients access and appropriate use of healthcare services.
  • Identify problems and resources to help clients solve their problems and empower them to navigate the healthcare system.
  • Work in community and hospital settings to ensure patients' needs are properly identified and addressed.
  • Collaborate with clinical social workers to ensure behavioral health needs are supported.
  • Complete daily medical appointment reminders to reduce missed appointments.
  • Liaise between interdisciplinary teams to avoid unnecessary hospital admissions.
  • Complete assessments to identify behavioral health and social determinants of health needs.
  • Work with the sickle cell transition population to ensure seamless care transitions.
  • Develop effective working relationships with patients and caregivers through engagement, collaboration, advocacy, decision-making, and problem-solving activities.
  • Attend transition program events as needed.
  • Assist patients in navigating the healthcare system.
  • Attend all scheduled trainings, workshops, and additional workshops as indicated by supervisor.
  • Identify and enroll eligible individuals in federal, state, and local health and human services programs.
  • Serve as a liaison between patients, families, communities, healthcare agencies, and specialists.
  • Provide guidance and social assistance to patients' families and community residents.
  • Enhance patients' ability to effectively communicate with healthcare providers.
  • Provide culturally and linguistically appropriate health education materials.
  • Provide referral, follow-up services, and schedule patient appointments as needed.
  • Assist patients and families in understanding and carrying out their healthcare providers' advice.
  • Follow up with patients to ensure they are receiving and adjusting well to their care plans.
  • Compile and track community resources available to and used by Jefferson's patient population.
Qualifications:
  • High School Diploma or GED.
  • Certified Community Health Worker training program preferred.
  • Prior experience in community outreach preferred.
