Join Greater Philadelphia Urban Affairs as a Family Navigator and play a vital role in supporting families of children and youth with behavioral health and/or autism spectrum disorder diagnoses.
About the Role:We are seeking a highly motivated and experienced individual to provide confidential support, mentoring, advocacy, and guidance to families navigating the complex child-serving systems in Philadelphia.
Key Responsibilities:- Establish and maintain strong relationships with families, community agencies, and stakeholders to ensure seamless coordination of services.
- Develop and implement individualized roadmaps for families, aligning with their self-determined needs and priorities.
- Provide education, training, and resources to families to enhance their capacity to meet their child's overall wellness needs.
- Collaborate with St. Christopher's Hospital staff and administration to provide advocacy and peer support to families and youth.
- Document all services and activities in the Electronic Healthcare Records/Program Database system.
- Parent/guardian with direct experience raising a child with a behavioral health and/or autism spectrum disorder diagnosis.
- Ability to use lived experience to support other families.
- Two years' experience/knowledge of Philadelphia child-serving systems and resources for children/adults with autism or behavioral and/or mental health challenges.
- One year's experience with fee-for-service programs, and/or knowledge of billable services, and/or documentation of services.
- Completion of Family Peer Support training within 3 months of hire.
- Child Abuse, FBI Fingerprint, and Criminal Background Clearances; all additional clearances and vaccinations according to St. Christopher's Hospital requirements.
- Knowledge of or training in Systems of Care and trauma-informed care.
- Experience with Child and/or Family Advocacy.
- Experience working with and understanding of billable service with the Medicaid system.
