We are seeking a highly skilled and compassionate Clinical Care Specialist to join our team.

About the Role:

The Clinical Care Specialist will be responsible for monitoring the health and well-being of our residents, providing leadership support, and assistance with resident care and services. Responsibilities include medication administration, communication with resident support services and families, and maintaining clinical quality assurance while leading and demonstrating elements of wellness core to Phoenix programs and services.

Key Duties and Responsibilities:

  • Identify ongoing needs and services of residents through assessment and Personalized Service Plans (PSPs).
  • E nsure proper follow-through and documentation for residents with changes in clinical care needs.
  • Support the Wellness Nurse with completing clinical sections of monthly wellness visits and PSPs.
  • Inform the Wellness Director and/or Executive Director of any changes in medication or service levels that may result in higher tier levels or potential move-outs.
  • Assist with obtaining weights and vital signs monthly for each resident prior to completion of monthly wellness visits.
  • Assist with resident care as needed.

Requirements:

  • Must be certified Medication Aide and/or meet state requirements for medication aide.
  • A minimum of one year of experience working with elders or disabled individuals preferred.
  • Demonstrated leadership capabilities required.
  • Knowledge and experience in Assisted Living industry and Dementia care or Skilled Nursing preferred.
  • Ability to work weekends, evenings, and flexible hours, available to customers at peak service delivery days and times.

Salary: $45,000 - $55,000 per annum, depending on experience and qualifications.
