We are seeking a skilled and compassionate PCP Nurse Practitioner to provide comprehensive care to patients in long-term care facilities.
**Estimated Salary: $80,000 - $120,000 per year**
**Key Responsibilities:**
- Provide healthcare services and coordinate patient care
- Work closely with a team of healthcare professionals
- Assess, diagnose, and treat residents' physical and mental well-being
- Conduct thorough patient assessments, including medical history reviews and physical examinations
- Order and interpret diagnostic tests
- Develop accurate diagnoses and treatment plans
- Prescribe medications and administer treatments
- Provide counseling to residents and their families
- Collaborate with the interdisciplinary team for seamless coordination of care
- Manage chronic conditions and monitor progress
**Locations:** Washington, DC, Pennsylvania, Delaware, Vermont, New York, Georgia, Florida, Massachusetts, New Jersey
**Requirements:
- Nurse Practitioner License
- DEA License
- 6 months experience in long-term care
