Job Summary:

Under the supervision of the RN Unit Manager or DON, the LPN will be responsible for providing nursing care to residents/patients, implementing and evaluating their plans of care, and supervising the performance of nursing assistants. The ideal candidate will possess excellent communication and interpersonal skills, be able to work effectively in a fast-paced environment, and demonstrate a commitment to delivering exceptional patient care.

Responsibilities:
  • Provide direct patient care, including administering medications, performing treatments, and monitoring vital signs.
  • Develop and implement individualized plans of care, taking into account the unique needs and goals of each resident/patient.
  • Supervise and evaluate the performance of nursing assistants, providing guidance and support as needed.
  • Collaborate with the healthcare team to ensure seamless care transitions and effective communication.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.
Requirements:
  • Licensed in the State as an LPN.
  • Current CPR certification.
  • Able to use necessary supplies and equipment, including feeding pumps, oxygen, treatment trays, and medication carts.
  • Able to calculate drug dosages and meet all health requirements imposed by law.
  • Ability to relate positively, effectively, and appropriately with residents, families, community members, volunteers, and other center staff.
  • Able to read, write, speak, and understand English, and demonstrate basic computer knowledge and ability with an aptitude to learn company applications.
