Job Summary

The Licensed Practical Nurse LPN will provide and document skilled nursing care to patients in their homes, under the supervision of a Registered Nurse, in accordance with the developed care plan and physicians orders.

Responsibilities
  • Deliver and coordinate quality patient care in compliance with physician orders.
  • Continuously observe and assess patient condition and care needs, reporting changes to the supervisor and/or physician as appropriate.
  • Document all activities, assessments, nursing actions, responses, and coordination of care in a timely manner.
  • Participate in and update the nursing care plan.
Requirements
  • Graduate of an accredited school of nursing.
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
Preferred Qualifications
  • One year of Long Term Care experience.
  • Six months of recent experience as a Licensed Nurse in a clinical care setting.
Physical Requirements

The successful candidate must be able to speak, write, read, and understand English, have visual and hearing acuity, and be able to climb stairs, lift 50 pounds, and travel.
