We are seeking a skilled clinician to join our House Calls team in Wisconsin, providing annual clinical assessments to patients in the comfort of their homes.
This role offers a unique opportunity to make a meaningful impact on patients' lives while enjoying a flexible schedule with no on-call, weekends, or holidays required.
As a member of our integrated medical team, you will work closely with patients, families, and healthcare providers to coordinate care and improve health outcomes.
Key Responsibilities:- Conduct comprehensive in-home assessments, including evaluations of past medical history, medication reconciliation, and vital signs.
- Perform evidence-based practice screenings, including point-of-care testing as appropriate.
- Identify diagnoses and develop care plans to address patient needs.
- Communicate findings with patients' primary care physicians to ensure seamless care coordination.
- Provide education and support to patients on disease management, medication adherence, and healthy lifestyle choices.
- Active NP or PA license in the state of WI or ability to obtain by start date.
- Active ANCC or AANP national certification in Family, Geriatrics, Adult, or Adult-Gerontology Primary Care certification.
- Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating various dwelling conditions.
- Access to reliable transportation for travel within licensed states.
- 1+ years of clinical experience in a similar setting.
- Experience in Gerontology, Cardiology, Internal Medicine, or Endocrinology.
- Home health care or home visit experience.
