We are seeking a highly skilled Advanced Practice Provider to join our team at Prevea Health.

Job Summary

The Advanced Practice Provider at Prevea Health is an advanced practice nurse (NP) or physician assistant (PA) prepared to deliver health care services to patients, families and groups emphasizing the promotion of health and prevention of disease under direct supervision and responsibility of physicians.

Key Responsibilities

  • Collaborate with patients to review their health background and determine physiological, emotional, and environmental needs, using this information to advise patients and their families on future health plans and illness prevention.
  • Conduct telephone calls back to patients requiring follow-up care or provide test results. Also provides health counseling and education to patients and their families about their healthcare and any follow-up instructions.
  • Maintain and review patient records, charts, and other pertinent information, documenting all details in the electronic medical record.
  • Collaborate with physicians and other providers to develop and implement patient care plans to ensure the best possible care and outcomes of patients.

Qualifications

  • Graduate of a master's level nurse practitioner program OR graduate from an accredited physician assistant program.
  • 1-3 years work experience in a clinical setting within a health care organization.

Preferred Skills and Abilities

  • Knowledge of the information and techniques required to diagnose and treat human injuries and diseases, including understanding symptoms, treatment options, drug properties and interactions, and preventive healthcare measures.
