Advanced Practice Provider

As a skilled Advanced Practice Provider, you will play a vital role in delivering high-quality patient care in our dynamic healthcare environment. Your expertise will be instrumental in assessing and managing the health needs of our patients, from stable long-term care to acute and episodic illnesses and injuries.
Key Responsibilities
- Conduct comprehensive health assessments for stable long-term patients, identifying potential health risks and developing personalized care plans.
- Assess and manage care for acute, minor, episodic illnesses and injuries, providing timely and effective interventions to promote optimal patient outcomes.
- Interview and advise patients and families on health and illness prevention strategies, empowering them to take an active role in their healthcare.
- Perform follow-up exams to monitor patient progress and adjust care plans as needed.
- Collaborate with clients and families to determine physiologic, emotional, and environmental needs and priorities, ensuring a holistic approach to patient care.
- Provide case management services to assist clients and families with arranging for or referring patients to needed services, promoting seamless transitions in care.
- Manage client and family health concerns through telephone triage and consultation, providing timely and effective support.
- Order diagnostic and laboratory tests, prescribe pharmacological agents, and implement nonpharmacological treatment modalities as needed, ensuring evidence-based practice.
- Maintain and review patient records, charts, and other pertinent information, ensuring accurate and up-to-date documentation.
- Perform patient callbacks, report diagnostic findings, and provide consultation to ensure timely and effective communication.
- Participate in call coverage, collaborating with physicians and other healthcare professionals to ensure seamless patient care.
Requirements
- 1-3 years of clinical experience as a licensed Advanced Practice Provider, with a minimum of two years of continuous performance in an urgent care or emergency room setting.
- Current licensure as a Registered Nurse in the State of Wisconsin.
- Valid Advanced Practice Provider licensure in the State of Wisconsin.
- DEA certification.
- Current certification in Basic Life Support (BLS), Advanced Cardiovascular Life Support (ACLS), and Pediatric Advanced Life Support (PALS), with certification occurring every two years.
