We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Chelsea Rehabilitation Center, a 84-bed skilled nursing center nestled in the heart of the Goochland community.

About the Role

As an LPN at Chelsea Rehabilitation Center, you will have the opportunity to provide exceptional patient care and work collaboratively with our interdisciplinary team to ensure the best possible outcomes for our residents.

Responsibilities
  • Provide respectful and compassionate care to all residents, families, and caregivers
  • Maintain resident confidentiality and accurate documentation in accordance with provided orders
  • Work collaboratively with the healthcare team to develop and implement individualized care plans
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments

Requirements

To be considered for this role, you must have:

  • A current, unencumbered, active license to practice as an LPN in the state of VA
