About the Role

We are seeking a skilled and compassionate Psychiatric Rehabilitation Worker to join our team at RHA Health Services. As a key member of our team, you will play a vital role in providing support to individuals with mental health or co-occurring issues in a site-based setting.

Key Responsibilities
  • Provide rehabilitative services, including recovery, stigma reduction, health and wellness, and personal development.
  • Develop, implement, monitor, and revise Functional Assessments and Person-Centered Plans.
  • Support individuals in developing various skill-building activities.
  • Monitor utilization of services to ensure effectiveness, appropriateness, and alignment with individualized plans.
  • Provide counseling and guidance to individuals to effectively deal with problems and develop positive relationships.
  • Assist with accessing community resources in accordance with individual goals for community involvement.
  • Prepare and complete necessary documentation, including 90-day reviews, activity notes, and incident reports.
  • Utilize safe crisis management techniques to manage situations involving agitated and/or hostile behaviors.
