At Springfield Medical Care Systems, Inc., we are seeking a Community Health Worker to join our Community Health Team. This role is a critical part of our mission to connect with and care for our community, ensuring everyone has access to health and wellness services.
The Community Health Worker will work closely with our primary care, behavioral health, and substance use treatment teams to provide screening, brief interventions, and connections to services addressing a range of needs, including clinical care, food insecurity, housing, and economic assistance.
Key responsibilities include:
- Providing outreach and support to individuals in need, fostering relationships and serving as a key point-of-contact for assistance.
- Conducting follow-up assessments and engaging individuals through motivational interviewing to identify priority needs and areas for intervention or follow-up.
- Identifying next steps to address findings, including referrals to appropriate levels of care within or outside our organization.
- Providing brief interventions as needed to address screening results, either directly or through referral.
- Collaborating with local health and social service organizations to enhance services and address systemic barriers to health and social services.
Requirements for the role include:
- Experience in direct service provision, preferably in a healthcare or social service setting.
- A passion for working within the community and a commitment to improving the health of individuals and communities.
- Excellent communication skills, including writing, listening, and organizing information.
- The ability to effectively and independently plan, organize, and coordinate access to health services.
Interested candidates should submit a cover letter addressing the scope of the role, in addition to completing the application. All new hires are required to show proof of COVID vaccination or request an exemption prior to starting work.
