Position Overview:

The role of the Clinical Pharmacist at Option Care involves delivering safe and effective pharmaceutical services to patients, adhering to professional standards and regulatory guidelines.

This position serves as an entry point into the realm of clinical pharmacy within alternative healthcare environments.

Key Responsibilities:
  • Evaluate patient eligibility for home care services and assess their appropriateness based on established criteria.
  • Develop, monitor, and assess patient-specific pharmaceutical care plans, providing recommendations for therapy adjustments as needed.
  • Modify pharmacy care plans based on patient information and deliver medication education to patients, caregivers, and healthcare providers.
  • Facilitate seamless pharmaceutical care across various care settings and oversee medication administration equipment for home care patients.
  • Address drug information inquiries, manage investigational drug products, and maintain compliance with legal, regulatory, and safety standards.
  • Uphold patient confidentiality and consistently adhere to organizational policies and procedures.
Essential Education and Experience:
  • Graduation from an accredited School of Pharmacy.
  • Possession of an active Pharmacy license from the State Board of Pharmacy.
  • A minimum of 1 year of experience in sterile IV admixture procedures or equivalent.
Required Qualifications:
  • Experience in customer service and proficiency in Microsoft Word, Excel, and PowerPoint.
Travel Expectations:

Willingness to travel occasionally for business purposes, if necessary.

Preferred Qualifications:
  • Completion of an ASHP accredited Pharmacy Residency Program or similar experience.
  • Experience in home care, hospital, or clean room settings.
Management Experience:

A minimum of 1 year of experience in team management and project coordination.
