About the Role

Ciena Healthcare is seeking an experienced and skilled MDS Nurse to join our team. As a key member of our care management team, you will be responsible for conducting assessments and care plan coordination for residents assigned to you.

Key Responsibilities:

  • Completes the Minimum Data Set (MDS), Care Area Assessments (CAAs), and care plans within regulated time frames.
  • Assesses residents through physical assessment, interview, and chart review.
  • Discusses resident care needs with caregivers, including physicians, nurses, social services, therapy, dietary, and activity staff.
  • Reviews information from hospitals, consults, and outside agencies and uses such information in the completion of the assessment and care planning.
  • Coordinates, identifies, and/or initiates significant change MDS.
  • Is prepared to conduct Prospective Payment System (PPS) meetings, maintaining MDS assessments per Medicare schedule and maintains PPS board for monitoring of Medicare days and Resource Utilization Groups (RUGs) utilization in the absence of the Care Management Coordinator.
  • Remains current with the American Association of Nursing Assessment Coordinators (AANAC) requirements.

Requirements:

  • Registered Nurse (RN) with a valid license.
  • AANC certification (RAC-CT) is a plus.
  • Knowledge of the Resident Assessment Instrument (RAI) process, including the principles of the Prospective Payment Process (PPS), strongly preferred.
  • Experience as an MDS Nurse.
