Practical Nurse Leader

Position Summary:
The LPN Licensed Practical Nurse will serve as a Medication Care Manager, responsible for administering medication and treatments in a safe and organized manner, conducting Service and Health Updates, and participating in the development of Individualized Service Plans (ISP)/Individualized Care Plans (ICP).
Key Responsibilities:
- Medication Administration: Administer medication and treatments according to prescribed orders and protocols.
- Service and Health Updates: Conduct regular Service and Health Updates to assess resident needs and develop individualized care plans.
- Documentation: Maintain accurate and detailed records of medication administration and other resident-related information.
- Communication: Collaborate with the Resident Care Director (RCD) and Wellness Nurse to ensure effective communication and coordination of resident care.
Requirements:
- LPN, LVN, or state/province specific licensed nurse credential.
- CPR and First Aid certification.
- Ability to work effectively in a fast-paced environment and prioritize multiple tasks and responsibilities.
- Excellent communication and interpersonal skills.
- Demonstrated ability to maintain confidentiality and handle sensitive information.
