Job Title: Licensed Practical Nurse (LPN)Job Description

We are seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) to join our Primary Care team. This role focuses on high-risk and chronic care management support and referral specialist support. Our healthcare services include primary healthcare, dental, and mental health services to children and adults, with an emphasis on populations living in or near public housing.

Responsibilities

+ Provide routine nursing care to patients.

+ Support high-risk and chronic care management.

+ Coordinate care for elderly and disabled patients, as well as those with chronic conditions such as diabetes, HIV, hypertension, and obesity.

+ Assist with referral processes and support specialist care.

+ Travel between Stamford and Greenwich as needed for patient care.

+ Work collaboratively within a multidisciplinary team.

+ Maintain detailed and accurate patient records using electronic health records (EHR).

Essential Skills

+ Current LPN licensure in Connecticut.

+ Minimum of 3 years of experience working as an LPN.

+ Experience in chronic care management and comfortable with care coordination.

+ Proficiency with electronic health records (EHR).

+ Excellent communication and interpersonal skills.

+ Ability to work collaboratively within a multidisciplinary team.

+ Bilingual in Spanish or Haitian Creole preferred.

Additional Skills & Qualifications

+ Experience in a primary care or outpatient setting preferred.

+ Knowledge of population health, care coordination, and referral processes.

+ Strong clinical skills in primary care and chronic disease management.

+ High attention to detail and ability to manage multiple responsibilities.

+ Strong organizational and time management skills.

+ Compassionate, patient-centered approach to care.

+ Ability to work independently and as part of a team.

Work Environment

Flexible schedule with office hours from Monday to Friday, 8am-5pm. The community health center serves patients of all ages, from infants to geriatrics. Training can range from 2-4 weeks, depending on the candidate's pace, and will include onboarding, orientation, and shadowing. The work involves standing and walking for up to 8 hours a day, stooping, kneeling, crouching, and lifting a minimum of 50 lbs. Excellent hand/eye coordination is required.

Pay and Benefits

The pay range for this position is $30.00 - $33.00/hr.
