Key Responsibilities
  • Assess patient responses to drug therapy and develop personalized treatment plans in collaboration with physicians.
  • Order and evaluate laboratory tests to monitor drug therapy effects and outcomes.
  • Initiate, modify, or discontinue medications for chronic disease states, such as hypertension, hyperlipidemia, and diabetes, in cooperation with the medical staff.
  • Monitor and manage pharmacotherapy requiring periodic adjustment due to specific or changing pharmacokinetic characteristics.
  • Administer prescription or non-prescription drugs according to established agreements or protocols.
  • Evaluate medical and medication histories to identify potential drug-related problems and adjust treatment plans accordingly.
  • Consult with other healthcare providers to discuss patient treatment needs and options.
  • Perform a full range of pharmacist procedures, including reviewing patient profiles and ensuring the accuracy of pharmaceutical products.
  • Provide medication and healthcare-related training to clinical staff on new medications, appropriate use, and other topics as deemed necessary.
  • Document significant medication interactions and pharmacy interactions with prescribers.
  • Conduct Adverse Drug Reaction (ADR) reporting and facilitate medical staff drug utilization reviews as needed.
Requirements
  • Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
  • Completion of a clinical pharmacy residency or fellowship accredited by the American Society of Health-System Pharmacists or American College of Clinical Pharmacy.
  • One year of full-time experience in an ambulatory care setting or completion of an ASHP-accredited Ambulatory Care Residency or Board Certification as a Pharmacotherapy Specialist.
  • Current, full, active, and unrestricted license to practice as a pharmacist.
