Job Summary:

The Clinical Coordinator Ambulatory Clinics Specialist supports the Cardiology and Advanced Heart Failure Clinic population by coordinating patient care at the local level, across sites and settings, and for the duration of the care continuum. This role requires critical thinking, excellent verbal and written communication, and the ability to work autonomously and collaborate with others.

Key Responsibilities:
  • Facilitate the development and provisioning of clinical resources
  • Assist with evidence-based practice and quality improvement initiatives
  • Collaborate with the interprofessional team to provide therapeutic patient and family-centered care
  • Collect and assess data related to patient health status to develop, execute, and evaluate the plan of care
Requirements:
  • Three years of experience as a nurse in a healthcare setting
  • Experience in the specialty is strongly preferred
  • Nursing licensure is required, and a Bachelor's Degree in Nursing is preferred
