Registered Nurse Care Coordinator

The Veterans Affairs, Veterans Health Administration is seeking a highly skilled and dedicated Registered Nurse to fill the position of Clinic Care Manager for the Ophthalmology Section. As a key member of the surgical team, the successful candidate will be responsible for coordinating patient care, ensuring seamless clinic operations, and providing exceptional patient experience.
Key Responsibilities- Coordinate patient care and ensure timely documentation
- Organize consults, coordinate equipment needs, and increase clinic utilization and throughput
- Provide education to surgical clinic patients as directed by Nurse Practitioners and Chief of Surgery
- Coordinate multiple consult/appointment follow-ups as needed
- Assist with resident teaching in clinic and manage daily workflow
- Coordinate inpatient to outpatient follow-up transition and oversee coordination of all clinic work-ups
- Triage all outpatient consults and coordinate image processing with other non-Ann Arbor VAMC Radiology Services when needed
- Provide communication link between administrative clinic support staff and other departmental support staff
- Oversee coordination of all outpatient follow-up visits and assist with various research projects if needed
- Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth of the United States, or the District of Columbia
- Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE)
- Basic Life Support (BLS) certification before the starting date of the position
- Registered Nurse Care Coordinator Experience
- Registered Nurse Clinic Care Coordinator Experience
- Registered Nurse Medical/Surgery Experience
