Job Summary

The Veterans Health Administration is seeking a Registered Nurse-Care Coordinator to work in the Ophthalmology Section of the Surgery Service. The successful candidate will be responsible for coordinating care for clinic patients and assisting with the clinic's daily workflow and operations. This position requires a working knowledge of the organization, clinic process, and knowledge of clinic procedure requirements.

Responsibilities

The Registered Nurse-Care Coordinator will be responsible for the following duties:
  • Assuring that appropriate documentation reflects patient assessments and interdisciplinary treatment plans in preparation for surgery or minor clinic procedures.
  • Demonstrating leadership in delivering and improving holistic care to adult and geriatric patients, through collaborative strategies, in compliance with the Michigan Nurse Practice Act, Medical Center, and national Veterans Affairs standards of practice.
  • Providing direct nursing care to individual and groups of patients under the immediate guidance and direction of the clinic's Nurse Manager, Nurse Practitioners (NPs), and ultimately the Associate Chief Nurse, Surgery and the Associate Director, Patient Care Services.
  • Coordinating changes in the clinic schedule with the patient and clinic staff; ensuring timely documentation; organizing consults, coordinating equipment or reusable medical equipment (RME) needs to increase clinic utilization and throughput.
  • Providing education to surgical clinic patients as directed by the NPs and Chief of Surgery; coordinating multiple consult/appointment follow-ups as needed and any other duties assigned by the supervisor.

Additional Responsibilities

The Registered Nurse-Care Coordinator will also be responsible for the following additional duties:
  • Making routine patient return phone calls; identifying red flag phone calls or patient issues to bring to the attention of NP or resident/attending staff.
  • Management of clinic profiles.
  • Management and updating of Service Agreement.
  • Consult completion management.
  • Encounter Action Required Report (EARR) management.
  • Attendance at facility Care Manager meetings.
  • Patient education in clinic.
  • Assist with resident teaching in clinic.
  • Manage daily workflow of the clinic.
  • Service specific duties within Scope of Practice.
  • Coordinate inpatient-to-outpatient follow-up transition.
  • Oversee coordination of all clinic work-ups.
  • Triage all outpatient consults.
  • Coordinate image processing with other non-Ann Arbor VAMC Radiology Services when needed.
  • Provide communication link between administrative clinic support staff and other departmental support staff.
  • Oversee coordination of all outpatient follow-up visits.
  • Assist with various research projects if/when needed.
  • Create and maintain standard operating procedure and competencies for all reusable medical equipment.
  • Provide back-up clinical service coverage for other surgical specialties to ensure adequate service coverage for staff illness, vacation, etc.

Requirements

The successful candidate will possess the following qualifications:
  • U.S. Citizenship.
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
  • Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE).
  • Experience as an RN will be evaluated to determine if the experience is of an acceptable level of quality with regard to the following five dimensions of nursing: Practice, Veteran/Patient Driven Care, Leadership, Professional Development, and Evidence-Based Practice/Research.

Preferred Experience

The Veterans Health Administration prefers candidates with experience in the following areas:
  • Registered Nurse Care Coordinator Experience.
  • Registered Nurse Clinic Care Coordinator Experience.
  • Registered Nurse Medical/Surgery Experience.

Physical Requirements

The successful candidate will be required to meet the following physical requirements:
  • Visual acuity, keen hearing, clear distinctive speech, and manual dexterity.
  • Potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing.
  • Transferring patients and objects may be required.
  • Exposure to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures.
  • Exposure to patients who are combative secondary to delirium, dementia, or psychiatric disorders.
