Job SummaryWe are seeking an experienced and compassionate Hospice RN/RN Case Manager to join our team. As a key member of our interdisciplinary team, you will work closely with patients, families, and healthcare professionals to develop and implement individualized care plans that promote comfort, dignity, and quality of life.
Responsibilities- Provide direct patient care services to hospice patients, including physical assessments, medication management, and emotional support.
- Collaborate with an interdisciplinary team to develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients.
- Act as a liaison between patients, families, and healthcare professionals to ensure seamless communication and coordination of care.
- Manage patient clinical records, including documentation of care plans, progress notes, and medication management.
- Participate in quality assessment and performance improvement programs to ensure high-quality care and patient outcomes.
To be successful in this role, you will possess:
- A current RN license in the state of requested employment (or eligibility to obtain)
- 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology
- Excellent communication and interpersonal skills
- A compassionate and empathetic approach to patient care

Estimated Salary: $73,000 - $95,000 per year, depending on experience and location.
