RN Case Manager

We are seeking a dedicated and skilled Registered Nurse to join our PRN Pool, which services Charlottesville, Virginia and surrounding counties. As a Registered Nurse in our hospice program, you will play a vital role in ensuring the comfort, dignity, and well-being of our patients.
Responsibilities- Conduct thorough nursing evaluations to develop personalized care plans in conjunction with physician's orders and interdisciplinary team members.
- Assess and re-evaluate patient needs, consulting with physicians and reporting changes to ensure optimal comfort and quality of life.
- Provide education and support to patients, families, and caregivers about disease progression, symptom management, and end-of-life care options.
- Collaborate with healthcare professionals to coordinate care and ensure a solid team approach to patient care.
- Maintain accurate and up-to-date documentation of patient assessments, care plans, and interventions using the Electronic Medical Record (EMR).
- Participate in interdisciplinary team meetings to contribute to the overall care plan and stay current with best practices in hospice care.
- Current and unrestricted Registered Nurse (RN) license in the state of Virginia.
- Minimum of 2 years of nursing experience, preferably in hospice, palliative care, or a related field.
- Knowledge of hospice regulations, practices, and standards of care, with a demonstrated ability to manage all aspects of terminal care, health team service coordination, documentation, and case management.
- Excellent clinical assessment and critical thinking skills, with emotional maturity and the ability to cope with family stressors and tolerance of individual lifestyles.
- Strong communication and interpersonal skills, with the ability to establish rapport with patients, families, and healthcare professionals.
- Compassionate and empathetic nature, with a genuine desire to provide comfort and support during challenging times.
- Ability to work independently and as part of a team, collaborating effectively with interdisciplinary professionals.
- Highly organized, self-motivated, and proficient in using electronic medical records (EMR) and other healthcare software.
