Home Health Registered Nurse Opportunities

We are seeking skilled Registered Nurses (RNs) to join our team as Home Health Registered Nurses in the West Michigan area. As a leading home care provider, Interim HealthCare offers a unique opportunity for RNs to provide one-on-one care to patients in their homes.

About the Role

As a Home Health Registered Nurse, you will be responsible for providing comprehensive care to patients with acute and chronic illnesses, injuries, or conditions. You will work collaboratively with a multidisciplinary team to develop and implement patient care plans that meet their individual needs.

Key Responsibilities
  • Conduct thorough assessments of patients' medical conditions and develop personalized care plans
  • Provide direct patient care, including medication management, wound care, and education on disease management
  • Collaborate with healthcare providers to ensure seamless transitions of care
  • Educate patients and their families on health promotion and disease prevention strategies

Requirements

To be considered for this role, you must have:

  • A current, active license as a Registered Nurse in the State of Michigan
  • At least one year of clinical experience as a Registered Nurse
  • Current CPR certification
  • Reliable transportation with a valid driver's license and car insurance
