Key Responsibilities- Assess patients' psychosocial status, strengths, and areas of need to develop effective care plans.
- Collaborate with the interdisciplinary team to provide education, support, and counseling to patients and their families.
- Work with patients to identify and address barriers to treatment adherence and quality of life.
- Provide referrals to community resources, such as home health services and vocational rehabilitation, to facilitate optimal treatment outcomes.
- Participate in quality improvement activities to enhance patient care and satisfaction.
- Masters in Social Work (MSW) degree from an accredited program.
- State required license to practice as a social worker.
- 0-2 years of experience in a healthcare setting, preferably in social work or a related field.
