Master's Caseworker

Job Summary:

Fresenius Medical Center is seeking a highly skilled Master's Caseworker to provide psychosocial services to patients treated by the facility. The ideal candidate will have a strong background in social work and be able to work effectively with patients, families, and healthcare teams to promote positive adjustment, rehabilitation, and improved quality of life.

Key Responsibilities:

• Participate in care planning in collaboration with patients and healthcare teams to identify effective interventions that will help patients meet rehabilitation and treatment goals.
• Provide monitoring and interventions for patients to adjust to dialysis and achieve optimal psychosocial status and quality of life.
• Provide supportive counseling services to patients as permitted within the scope of their clinical training and state license.
• Assist patients with accessing community resources, including home health services, vocational rehabilitation, and advance directive forms.
• Collaborate with healthcare teams to promote patient satisfaction and quality of life, and actively participate in process improvement activities.

Requirements:

• Masters in Social Work
• Strong background in social work and experience working with patients with chronic illnesses
• Excellent communication and interpersonal skills
• Ability to work effectively in a team environment
