About the Role

Silverado Hospice is seeking a Part-time Palliative Licensed Clinical Social Worker to join our team in the North San Diego area. Responsibilities
  • Conduct assessments, develop care plans, and document bereavement activities
  • Design and distribute materials for families receiving bereavement services
  • Develop educational programs on loss and grief for families and staff
  • Evaluate the bereavement program quarterly and annually
  • Provide community consultation and education on hospice services
  • Maintain ongoing education through academic studies and workshops
  • Attend in-services, staff meetings, and case conferences
  • Report patient condition changes to the RN Case Manager
  • Observe and address safety hazards in patient and organizational environments
  • Assist health team in understanding emotional factors related to terminal illness
  • Assess suicide risk, mental capacity, and elder/dependent adult abuse
  • Aid in advance directives, mortuary arrangements, and financial planning
  • Provide information and referrals to community resources and legal entities
  • Advise and counsel staff, patients, families, or agencies in emergency situations
Requirements
  • Master's degree in Social Work is required
  • Documented evidence of training in dealing with grief and loss
  • Valid driver's license, auto insurance, reliable transportation, and willingness to drive to patient's location
