Job Summary

We are seeking a skilled and empathetic Hospice Registered Nurse (RN) to join our team as a Case Manager. As a key member of our interdisciplinary team, you will provide direct patient care services, empower patient caregivers, and collaborate with healthcare professionals to ensure the best possible outcomes for our patients.

Responsibilities
  • Provide direct patient care services, including assessing patient needs, developing and implementing care plans, and evaluating patient progress.
  • Empower patient caregivers by educating them on care for their loved ones, promoting independence and self-care.
  • Collaborate with healthcare professionals, including physicians, nurses, and social workers, to ensure comprehensive care and support for patients and their families.
  • Maintain accurate and detailed patient records, ensuring compliance with regulatory requirements and company policies.
  • Participate in quality assessment and performance improvement initiatives, contributing to the development and implementation of evidence-based practices.
Requirements
  • RN license in the state of employment (or eligibility to obtain).
  • 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology.
  • Current driver's license and automobile insurance.
  • Current CPR certification.
