Job Title: LPN - Boston, MA, United States

Fenway Health is seeking a skilled Licensed Practical Nurse (LPN) to join our team at our outpatient community health center in Boston, MA. As an LPN, you will play a vital role in providing high-quality patient care and supporting our mission-driven nurses in delivering compassionate care to our patients.

Key Responsibilities:
  • Provide specialized nursing support for patients and medical providers, including phone support and triage.
  • Assist with patient care under the direction of medical providers and/or under nursing scope of practice.
  • Administer medications, vaccinations, and other therapeutic interventions as per medical provider order.
  • Coordinate follow-up care for acutely ill patients, including primary care and specialty visits, VNA, Infusion companies, and prior authorization for services.
  • Document services in medical records, including all appropriate activities and plans.
Requirements:
  • Current licensure as an LPN in the Commonwealth of Massachusetts.
  • Current CPR certification.
  • Ability to work harmoniously with diverse groups of individuals.
Preferred Qualifications:
  • IV insertion/phlebotomy skills.
  • Supervisory experience.
  • 1-year of experience in an ambulatory care setting.
  • Experience with patients with HIV and electronic medical records.
