LHC Group is seeking a skilled Physical Therapist to join our team in South Hackensack, New Jersey.
Job SummaryWe are looking for a dedicated Physical Therapist to provide high-quality care to our patients in the comfort of their own homes. As a Physical Therapist with LHC Group, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
Responsibilities- Evaluate patient care needs and develop individualized treatment plans to improve functional status and overall well-being.
- Conduct thorough assessments and evaluations to identify patient needs and develop effective treatment plans.
- Collaborate with healthcare teams to ensure seamless care coordination and communication.
- Provide education and training to patients and their families on proper care and treatment techniques.
- Current Physical Therapy licensure in the state of New Jersey.
- Current CPR certification.
- Must have a current driver's license and vehicle insurance, and access to a dependable vehicle.
