Job Summary

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments in the comfort of patients' homes, providing unmatched insight into their overall health and well-being.

Key Responsibilities
  • Conduct thorough adult and geriatric assessments, medical history, diagnosis, and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
  • Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
  • Collaborate with Primary Care Physicians (PCPs) on patient education and provide follow-up.
  • Provide services in various venues, including home visits, skilled facility visits, and telehealth visits.
Requirements
  • Master's degree required or commensurate experience and satisfactory completion of NP licensure.
  • Current RN and NP or PA licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
  • Board certification by the NCCPA, AANP, ANCC, or AACCN in a Matrix-approved specialty.
  • Current BLS, ACLS, or CPR certification.
  • Minimum of 1 year experience as a Nurse Practitioner/Physician Assistant, with the ability to travel in-state or out-of-state.
  • Strong computer skills and familiarity with EMR software.
  • Excellent verbal and written communication skills with patients, clients, and colleagues.
  • Positive and outgoing personality, with a willingness to be a team player and adapt to frequent change.
Travel Requirements
  • Ability to travel, valid state driver's license, and ability to drive a car, with proof of adequate automobile insurance coverage for the state of residence.
  • Travel required to meet patients where they live.
