Hueman Risk Adjustment Solutions is actively seeking qualified Nurse Practitioners and Physician Assistants to join our team of healthcare professionals. As a member of our team, you will have the opportunity to work in a unique and rewarding environment, conducting in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in and around Shakopee, MN.

This part-time opportunity offers a flexible schedule, allowing you to balance your professional goals with your personal life. Our comprehensive home assessment training will ensure you are equipped to provide high-quality care to our members.

Responsibilities
  • Travel to and from patients' homes to conduct comprehensive, non-invasive physical assessments
  • Review medical history and ensure accurate and timely documentation
  • Conduct medical reconciliation to ensure patient safety and compliance
  • Provide basic patient education on wellness and preventative healthcare measures
Qualifications and Experience

To be considered for this opportunity, you must have a Master's Degree or commensurate experience and a satisfactory completion of NP/PA license. Additionally, you must have an active State NP/PA license, active National board certification, and current BLS certification. 12 months of experience as a paid advanced clinical provider is preferred but not required.
