Position Overview: Nurse Practitioner

Monogram Health is seeking dedicated Nurse Practitioners and Physician Assistants who are passionate about enhancing the lives of patients. As an Advanced Practitioner, you will play a vital role within a cohesive Care Team that includes a Registered Nurse and a Social Worker. Our patient population often faces multiple chronic conditions, and your expertise will be essential in helping them improve their quality of life and manage kidney disease effectively.

Your Contribution

In this role, you will provide personalized and compassionate medical care primarily to individuals with Chronic Kidney Disease (CKD) and End-Stage Renal Disease (ESRD). Responsibilities include maintaining accurate patient records, scheduling follow-up appointments, and delivering care that prioritizes patient well-being. Key Responsibilities
  • Conduct comprehensive assessments, including annual wellness exams, in both home and virtual settings
  • Educate patients and families about available resources and programs to support healthier living
  • Document clinical findings, including chief complaints, diagnoses, medical history, and treatment plans
  • Coordinate care with primary care providers, specialists, and ancillary services
  • Ensure timely completion of all necessary documentation
  • Maintain high standards of care as defined by the practice
  • Work remotely within the state of employment while attending periodic training and meetings
  • Deliver evidence-based care aimed at reducing hospitalizations and enhancing patient satisfaction
  • Prescribe medications, order diagnostic tests, and collaborate with the Monogram physician
  • Achieve improved patient outcomes as measured and reported regularly
  • Facilitate patient education on preventive care and assist with scheduling necessary tests
  • Participate in integrated care team meetings
  • Assess and develop treatment plans based on patient health status
  • Implement and evaluate care plans effectively
  • Perform additional duties as assigned
Qualifications
  • Bilingual in English and Spanish preferred
  • Active and unrestricted licenses as a Registered Nurse and Nurse Practitioner or Physician Assistant
  • Board certification relevant to licensure (NP: ANCC/AANP; PA: NCCPA)
  • Current DEA certification
  • Ability to work independently and practice autonomously
  • Access to reliable transportation and valid driver's license
  • Proficiency with medical instruments and healthcare technology
  • Effective communication skills for interactions with patients, families, and healthcare professionals
  • Understanding of federal and state regulations, including HIPAA and OSHA
  • Minimum of two years of direct patient care experience required
  • Experience in Managed Care or Health Plans is a plus
  • Familiarity with conducting wellness visits in home or virtual settings
