We are seeking a nurse practitioner to join our team, focusing on hospice and palliative care. This role offers the chance to work autonomously while collaborating with a dedicated medical team to ensure comprehensive care from the initial assessment through to discharge.
Position Highlights:
Work Environment:
- Deliver hospice and palliative care to patients
- Operate independently in an Advanced Practice Provider role
- Collaborate with the Palliative Care Medical Team to provide tailored care
Key Responsibilities:
- Conduct thorough assessments, including evaluations of disease progression, functional changes, nutritional status, and pain management
- Prescribe necessary medications, order and interpret laboratory tests, and arrange for treatments or medical equipment
- Participate in regular meetings with the Palliative Medical Director, at least biweekly
Additional Information:
- Mileage reimbursement is provided
- Work schedule: Four 10-hour shifts with no weekend commitments
Qualifications:
- Minimum of 2 years of Nurse Practitioner experience required
- ACHPN certification is preferred
- Active BLS and DEA certifications are mandatory
