We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a Medication Care Manager at Sunrise Senior Living. As a Medication Care Manager, you will play a vital role in ensuring the highest quality of care for our residents by administering medications and treatments in a safe and organized manner.
Key Responsibilities- Medication Administration: Administer medications and treatments to residents in accordance with their individualized care plans and medical orders.
- Documentation and Communication: Accurately document medication administration and resident interactions in the resident's medical record, and communicate effectively with residents, families, and healthcare team members.
- Resident Care: Provide high-quality care to residents, including conducting Service and Health Updates, participating in daily Cross Over meetings, and contributing to the development of Individualized Service Plans (ISPs).
- Risk Management and General Safety: Partner with the community team to ensure compliance with OSHA and other regulatory requirements, and maintain a safe and healthy environment for residents and staff.
- LPN License: Current LPN license in good standing.
- Certifications: CPR and First Aid certifications, and any other certifications required by state or province.
- Experience: Previous experience working with seniors, preferably in a long-term care setting.
- Skills: Strong communication and interpersonal skills, ability to work independently and as part of a team, and ability to make sound judgments and decisions.
