Speech Therapist

Maxicare Select, a Legacy Care Partners, Inc company, is seeking a compassionate and dedicated Speech Therapist to provide quality home health care services to patients in need. As a member of our Home Health team, you will utilize your clinical expertise to assess, teach, and deliver therapy interventions and treatment directed by the physician and established in the plan of care.
Key Responsibilities:
- Perform initial and ongoing assessments to evaluate patients' health status and develop individualized care plans
- Administer and interpret tests and measurements to determine functional goals and outcomes
- Provide written evaluations and plans of care defining speech therapy needs, goals, progress, and expected outcomes
- Collaborate with other healthcare team members to provide coordinated and holistic care to patients
- Ensure accurate and timely documentation of patient care and progress
Requirements:
- Valid and current ST license in the state of Florida
- Minimum of 1 year of home health or related experience
- Strong clinical and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work independently and prioritize tasks efficiently
