Diabetes Care and Education Specialist

Job Summary
MaineHealth is seeking a skilled Diabetes Care and Education Specialist to provide direct patient education for individuals with diabetes and their families. This role involves preparing patients for safe diabetes self-care after discharge and collaborating with healthcare professionals to develop clinical diabetes management protocols.
Key Responsibilities
- Provide patient education on diabetes self-care and management
- Collaborate with healthcare professionals to develop clinical diabetes management protocols
- Act as a resource for healthcare professionals and patients on diabetes-related topics
- Assist with outcomes data analysis and quality improvement initiatives
Requirements
- Current license and registration to practice as a Registered Nurse in the State of Maine
- Currently certified as a diabetes educator
- Education: Knowledge of nursing theory, practice, and education sufficient to coordinate care for diabetic patients
- Experience: Two years progressively more responsible related experience in the area of diabetes education
