About the Role

We are seeking an experienced RN to join our Home Dialysis team as a RN CAP 1. In this role, you will be responsible for providing high-quality patient care, coordinating treatment protocols, and participating in staff training and education initiatives.

Key Responsibilities
  1. Patient Assessment and Care Planning: Conduct comprehensive patient assessments, develop personalized care plans, and coordinate treatment protocols to ensure optimal patient outcomes.
  2. Interdisciplinary Collaboration: Work closely with patients, family members, and healthcare professionals to ensure seamless care transitions and promote a positive work environment.
  3. Staff Training and Education: Participate in staff training, education, and quality improvement initiatives to enhance patient outcomes and improve departmental efficiency.
Requirements
  • Current RN licensure in the state of employment.
  • Minimum of 12 months of experience as a RN in a nephrology setting.
  • Successful completion of CAP requirements or willingness to participate in educational programs.
