Job Summary

The Member Navigator plays a vital role in guiding members with chronic health issues, gaps in care, or difficulties engaging with the healthcare system. This position requires strong communication and interpersonal skills to establish trusting relationships with members, assess barriers to care, and facilitate connections between members, healthcare providers, and community resources.

Key Responsibilities
  • Engage members telephonically and other appropriate methods to establish a trusting relationship and assess barriers to care.
  • Conduct member screenings and refer to case management if clinical needs are identified.
  • Maintain accurate documentation of member contact, referrals, and outcomes.
  • Perform quality assurance reviews and trend studies to identify areas for improvement.
  • Build partnerships with healthcare teams, providers, and community resources to ensure timely delivery of quality care.
Requirements
  • Associate's degree or 2 years of work experience above the minimum qualification.
  • 3 years of experience in a related field, such as community health advocacy or navigation.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work autonomously with minimal supervision in a fast-paced environment.
