Care Transition Navigator PRN

At Methodist Health System, we are seeking a highly skilled Care Transition Navigator PRN to join our team. As a Care Transition Navigator PRN, you will play a critical role in coordinating activities that promote quality outcomes, patient throughput, and discharge planning while supporting a balance of optimal care and appropriate resource utilization.

Key Responsibilities:
  • Coordinate patient care transitions to ensure seamless handoffs and optimal outcomes
  • Identify potential barriers to patient throughput and quality outcomes, and develop strategies to mitigate them
  • Collaborate with interdisciplinary teams to develop and implement effective discharge plans
  • Communicate clearly and openly with patients, families, and healthcare teams to ensure informed decision-making
  • Build relationships with patients, families, and healthcare teams to promote a collaborative environment
Requirements:
  • Bachelor's degree in Social Work, Master's degree in Social Work, or Registered Nurse with BSN preferred
  • Hospital case management experience preferred
  • LMSW/LBSW, or RN as licensed by the Texas Board of Examiners, CCM or ACM preferred
  • 1 Year related work experience
  • Ability to prioritize multiple tasks in a fast-paced work environment
  • Ability to periodically flex work schedule as indicated by client or hospital needs
  • Ability to develop and maintain good working relationships with all levels of staff
