We are seeking a highly skilled and compassionate Patient Care Coordinator - Home Health Specialist to join our team at All American Home Care LLC. As a key member of our care coordination team, you will play a vital role in ensuring the highest level of care for our patients in the comfort of their own homes.
Key Responsibilities:
- Develop, monitor, and evaluate home health care plans for patients to ensure their needs are met and exceeded.
- Coordinate with home health aides to ensure a high level of care is provided to patients.
- Schedule agency home health care aides and manage last-minute call-outs to ensure continuity of care.
- Develop and maintain an on-call pool of agency aides with various shifts in coordination with Human Resources.
- Communicate effectively with referral sources and Case Managers to provide exceptional customer service.
- Match clients with the most suitable caregivers based on their needs and preferences.
- Manage a caseload of 100+ patients, ensuring timely and effective care coordination.
- Review care plans with patients and caregivers to ensure understanding and satisfaction.
- Resolve grievances and complaints from caregivers and clients in a fair and timely manner.
- Monitor and address overutilization of hours to ensure efficient use of resources.
- Report personnel performance issues to management.
- Contribute to team efforts by accomplishing related results as needed.
Requirements:
- High school graduate or equivalent.
- 2+ years of experience in a Home Care or Service Coordination role.
- Demonstrated ability to maintain strict confidentiality.
- Proven ability to generate leads and monitor referrals.
- Strong typing and computer skills.
- Excellent communication, public relations, and follow-up skills.
- Experience with HHA Exchange and EVV.
- Bilingual English/Spanish preferred.
