Job Description

We are seeking a highly skilled and compassionate Care Designer to join our team at Nurse Next Door. As a Care Designer, you will play a vital role in delivering exceptional client care and driving business results through consultation conversion and ongoing client care experience.

Key Responsibilities:

  • Conduct Caring Consults to assess client needs and advise on care plans, achieving a conversion goal of 90%

  • Develop and promote high-quality, innovative customer service to drive business interest and establish long-term relationships with community leaders and professional organizations

  • Act as an advisor in Caring Consults to ensure clients and their contacts have all necessary information

  • Attend First Visits with caregivers to ensure client needs and care plans are executed as per the Service Agreement, setting and role-modeling expectations

  • Conduct supervisory visits to ensure caregiver fit and quality of care, delivering a premium client experience and impacting Net Promoter Scores of 9 or higher

  • Develop and update individualized care plans as required, educating clients on self-care and evaluating client response to care provided

  • Monitor risk issues and analyze trends, adapting care as needed, and coordinating care to implement clinical solutions and evaluate client outcomes based on client goals

  • May need to step in as a caregiver if needed, ensuring compliance with national, state/provincial legislative requirements in areas such as health, employment, and occupational health & safety

  • Responsible for approving all visits and premiums in preparation for each payroll and billing cycle, ensuring data integrity and completion in the scheduling software

  • Collaborate with the Care Services Center team to communicate changes in client requirements and caregiver assignments, ensuring client coverage is secured and scheduling is accurate and timely

Requirements:

  • Bachelor's Degree in Nursing, Social Sciences, or related field preferred, but not required

  • CNA or CMA Certificate, CPR and First Aid Certification required

  • Caregiving experience is an asset, with a nursing background considered an asset

  • Managerial experience of at least 5 years, with demonstrated experience setting and achieving goals

  • Strong computer skills, proficient in G-Suite: Gmail, Drive, Docs, Calendar, and exceptional written and verbal communication skills

  • Intentional organization, planning, time management, and multitasking skills, with a clean driving record and insured vehicle

  • Able to get along with multiple personality types, detailed and organized, and able to multitask and operate under pressure

  • Focused and a go-getter attitude, with a caring nature and passion for people
