Job Title: Home Health Physical Therapist

We are seeking a skilled and compassionate Physical Therapist to join our team at Alternate Solutions Health Network LLC. As a Home Health Physical Therapist, you will provide high-quality care to patients in their homes, helping them to recover from illness or injury and maintain their independence.

Job Summary:

The successful candidate will have a current license to practice as a Physical Therapist in the state of employment and a valid driver's license. You will be responsible for providing comprehensive skilled services as ordered by the attending physician, including observing, recording, and reporting patient reactions, changes, and progress. You will also be responsible for educating patients and their family members on proper use of equipment and ensuring the safety of the patient.

Responsibilities:
  • Plan of Care: Develop and implement individualized plans of care for patients, including providing skilled physical therapy as ordered by the attending physician.
  • Patient Outcomes: Treat patients to relieve pain, develop or restore function, and maintain maximum performance using physical means.
  • Minimize Patient Risk: Help decrease re-hospitalizations by front-loading visits for high-risk patients and teaching patients on preventative measures.
  • Family Educator/Advocate: Counsel patients and family members on disease/injury management and instruct them on proper use of equipment.
  • Supervision: Oversee and supervise therapy services provided by PTAs and nurse assistants.
  • Communication/Collaboration: Communicate with PTAs and nurse aides on supervisory visit results.
  • Policies: Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
  • Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations.
  • Operations: Attend in-service training and mandatory agency meetings.
Requirements:
  • Current license to practice as a Physical Therapist in the state of employment.
  • Valid driver's license and auto insurance in your name as a driver.
  • Capable of all physical demands.
