Job Title: Physical Therapist Home Health

Alternate Solutions Health Network LLC is seeking a skilled Physical Therapist to join our team in providing high-quality home health care services to patients in Dayton, Ohio.

About the Role:

We are looking for a compassionate and dedicated Physical Therapist to work with our interdisciplinary team to provide comprehensive skilled services to patients in their homes. As a Physical Therapist, you will be responsible for evaluating patients, developing individualized plans of care, and implementing treatment plans to help patients achieve their goals.

Responsibilities:
  • Provide skilled physical therapy services to patients in their homes, as ordered by the attending physician.
  • Evaluate patients and develop individualized plans of care to address their physical therapy needs.
  • Implement treatment plans to help patients achieve their goals, including exercises, stretches, and other interventions.
  • Communicate with patients, families, and healthcare providers to ensure seamless care coordination.
  • Document all aspects of patient care, including progress notes, treatment plans, and discharge summaries.
Requirements:
  • Current license to practice as a Physical Therapist in the state of Ohio.
  • Minimum of one year of experience as a Physical Therapist in an acute setting.
  • Home care experience is desired.
  • Ability to effectively communicate with patients, families, and healthcare providers.
  • Knowledge of therapy best practices and ability to make appropriate physical therapy judgments.
