Job Title: RN-Home Care

We are seeking a skilled and compassionate RN-Home Care professional to join our team at Mercy. As a RN-Home Care, you will provide high-quality patient care in the home setting, working closely with patients and their families to ensure the best possible outcomes.

Responsibilities:
  • Provide day-to-day patient care in the home setting, including case management, total care of multiple patients with different levels of acuity, and patient teaching.
  • Assist with admissions to home care services, post-hospitalization assessments, recertifications, discharges, and patient transfers.
  • Administer IV therapy, access site care, and general skilled nursing according to policy and procedure.
  • Document patient care and progress in the Epic computer system.
  • Perform duties and responsibilities in a manner consistent with our mission, values, and Mercy Service Standards.
Requirements:
  • Graduation from an accredited school of nursing.
  • RN license in Arkansas or Oklahoma.
  • Valid drivers license in Arkansas or Oklahoma with a good driving record.
  • Three years of nursing experience, with a minimum of one year in a hospital setting and preferably in home care or the equivalent.
  • CPR certification.
  • Ability to lift 50 pounds, transfer patients weighing at least 100 pounds, and perform other physical demands of the job.
Preferred Qualifications:
  • Basic computer skills.
  • Wound Vac application experience.
  • Telehealth monitoring experience.
